| The certification and
enrollment process is facilitated by OBH and Medicaid’s Provider
Enrollment unit, Unisys. OBH conducts the initial application review and
an on-site review to ensure the applicant meets program and staffing
requirements to participate in the MHR program. If the applicant meets
all of the certification requirements, the application is submitted by
to Provider Enrollment to complete the enrollment process. Provider
Enrollment determines if the applicant is approved to become a Medicaid
provider and issues the provider number which is required to participate
in the MHR program.
The MHR certification and enrollment process is applicable to the
following:
- Applicants requesting initial certification and enrollment as a
provider to offer core MHR services.
- Certified providers requesting a change in ownership.
The Bureau has an established process for applicants to request
certification and enrollment to provide core MHR services.
